Epic brake fail...

It more my fault than anything...Drove out to the lake on Sat night to do the fire thing. It’s a dirt road with a few hills to climb some that need 4x4. On the way out from the trail I noticed that my breaks were a little soft. I pulled over before hitting the main road and took a look. I could see anything missing or leaking so I took it slow and drove home.
Sun morning I went to pull the truck in garage to take a look and it wouldn’t back up. I look around and see a nice groove on the inside of my rim. The upper bolt for the caliper fell out… again. This is the second time in a month that this has happened.
Last time it happened I picked up a new bolt from the dealer and made sure to LT them in. I checked them a week after I put them in and they were good.
Im not sure what to do now. I have new bolts in again and will be checking them again but to have the same side fall out has to be more than a coincidence…

someone must have saw you on the epiphany toilet and is trying to off you

That's really odd.
So do feel a vibration up front that could be working on it? Can't think of anything for sure. A wheel bearing perhaps?

I know the rotors are a little warped again... Going to pick up some new ones.
